A Retrospective Comparison of Phaco-stent vs. Phaco-goniotomy in Glaucoma Patients

The purpose of this study is to compare surgical outcomes of phaco-goniotomy or phaco-stent in patients with mild to moderate glaucoma and visually significant cataracts. The design is a single-center, retrospective, comparative case series. 104 patients were in the stent group and 51 were in the goniotomy group. Statistical significance was determined at a p <0.01. There was a reduction of IOP criteria to determine surgical success or failure. Secondary outcome measures were intraocular pressure (IOP) and glaucoma medication use. The cumulative probability of failure at four years post surgery were Hydrus Microstent 0.47 (95% CI: 0.36 to 0.57) versus KDB Goniotomy 0.40 (95% CI: 0.26 to 0.53) (p = 0.65). The number of glaucoma medications were Hydrus (n=104) 2.1 ± 1.2 vs KDB (n=51) 1.7 ± 1.5 at baseline and Hydrus (n=104) 1.2 ± 1.3 vs KDB (n=51) 1.1 ± 3.8 at last follow up. The estimated mean difference (KDB – Hydrus) was -0.5 (99% CI: -1.1 to 0.1; p = 0.05) at baseline and -0.2 (99% CI: -0.7 to 0.4; p = 0.39) at last follow up. The difference in results for phaco-stent vs. phaco-goniotomy were not significant. At four years there was no difference in surgical failure between the Hydrus and KDB groups. In addition, there is no difference between intraocular pressure or medication burden.
